When you first meet with us, depending on your financial situation, you must provide certain documents. To find out which ones, see the list below and click on the description that best fits your current situation. This is not an exhaustive list and some additional documents may be required. If you have any doubts about what to bring, please contact your local legal aid office.

BENEFITS – INCOME STATEMENTS
– Regular benefits: claim booklet from the Ministry of Labour and Social Solidarity for the current month or, failing that, proof that you receive last resort assistance, and the amount received
– Persons with a severely limited capacity for employment: claim booklet from the Ministry of Labour and Social Solidarity for the current month, or, failing that, proof that you receive benefits for persons with a severely limited capacity for employment, and the amount received
– Employment assistance: proof that you receive employment assistance benefits, and the amount received.
PERSONAL INFORMATION:
– Social insurance number
– Address and phone number
– Health insurance card
– If you are not a Canadian citizen: official document proving your status in Canada
– If you are a native person: your band number

PROOF OF INCOME FOR THE LAST YEAR PRIOR TO YOUR REQUEST
– Provincial income tax return
– Provincial notice of assessment
PROOF OF INCOME FOR THE CURRENT YEAR
– Last pay stub, with an indication of the cumulative amount of gross earnings for the current year
– Record of employment
– Names and addresses of all sources of income
– Proof of amounts received as tips for the current year (if not indicated in your pay statement)
CHILD OUR SPOUSAL SUPPORT RECEIVED
– Copy of the latest judgement concerning child or spousal support
– Detailed statement from the Ministry of Revenue confirming the sums received
INCOME REPLACEMENT (CNESST, SAAQ, QPIP, VCI)
– Proof or statement showing the amounts received (for QPIP: gross amounts), and indicating the date of the beginning and end of the payments
EMPLOYMENT INSURANCE
– Proof or statement confirming the number of weeks of benefits allowed, the gross amount received, as well as the date of the beginning and end of the payments
ADMISSIBLE INCOME DEDUCTIONS

– Copy of the latest judgement concerning child or spousal support
– Detailed statement from the Ministry of Revenue confirming the sums paid
CHILD CARE FEES
– Receipt for eligible provincial tax credit amount

FEES TO ALLEVIATE A SERIOUS PHYSICAL OR MENTAL DEFICIENCY
– Proof justifying expenses that were non reimbursed by an insurance company or a government plan

– Bank statements for all your accounts (current, savings, investment, term deposits, RESP, TFSA, RRSP, etc.) for the month prior to your request, and the balance for all your accounts at the time of your request
